Cutting-edge technology to address social determinants of health aims to serve as a national model for improving health and quality of life

A newly unveiled integrated platform will facilitate collaboration among healthcare providers and community-based organizations (CBOs) to create ‘whole person health’ teams for underserved and at-risk populations.
