In-home health evaluations

Our nationwide network of more than 10,000 licensed providers engages health plan members at home to provide diabetic eye exams, peripheral artery disease testing, bone density testing and lab collection, including HbA1c, low-density lipoprotein, microalbumin, and fecal occult blood tests.
Comprehensive medication review
Drive positive therapeutic outcomes and Star ratings with our efficient medication therapy management solution. During an In-home Health Evaluation, Signify Health clinicians thoroughly review members’ medications and evaluate adherence, as required by CMS. We discuss their prescription regimens, answer questions, and provide educational resources.
