Preparing for your visit
This visit focuses on managing the symptoms related to your health condition, discuss any concerns you may have, screen you for any complications, and help identify any additional resources you may need. You can prepare for your Focused Visit by doing these things before your visit:

Make sure your medications and any self-testing devices (if applicable) are available.

Because eye care is important, the clinician may want to examine your eyes. If so, your eyes will be checked with a special camera. Dilation will not be required.

Write down any questions or concerns you have about your managing your health.

Make a list of the foods you usually eat in a typical week. This will help the clinician understand your diet and eating habits.

You may be required to provide a urine sample and have blood drawn. To prepare, drink plenty of water before your visit.

We may ask that a trusted adult (spouse, adult child, or caregiver) join you for the visit.
After your visit
You will receive a personalized care plan following your Focused Visit. The results from your visit will be shared with your regular primary care provider (PCP). In your next regularly scheduled appointment, your PCP will have more information to assess your health care needs better. It is important to maintain your regularly scheduled visits with your PCP.

Do I have to fast for this visit?
No, if a blood and urine sample is required, it does not require fasting.
If I am eligible for an In-Home Health Evaluation, could I also get a Focused Visit if I need it?
Yes. If you are eligible for an In-Home Health Evaluation, you can also receive a separate Focused Visit.
How will I know if I should have a Focused Visit?
If you have been diagnosed with certain conditions, you may be eligible for a Focused Visit. This visit gives you more opportunity to discuss any symptoms you may be experiencing and learn more about managing your illness.
If I have a Focused Visit, will I see the same clinician as I did for my In-Home Health Evaluation?
In some cases, the same clinician you had for your In-Home Health Evaluation will perform the Focused Visit, but not always. If you have a different clinician, they will have access to your prior visit records.
How long does a Focused Visit take?
The Focused Visit gives you more time to talk with a licensed clinician about your specific condition. The visit will take about 30-60 minutes, giving you and the clinician time to address any questions and perform necessary screenings.
How much does the Focused Visit cost?
There is no additional cost to you.
Do I have to have a Focused Visit?
You are not required to have a Focused Visit, but it is a no-cost benefit of your health plan, scheduled at your convenience in your home.
Why do I need to have another appointment with my regular doctor if I’ve already had a Focused Visit?
The Focused Visit is a benefit or an additional service offered to you. It does not replace your regular visits with your PCP and health care team. This in-home visit will help your PCP understand your health better and give you more time to ask questions and learn how to manage your health during your next PCP visit.
