FARMINGTON, CT – (April 13, 2022) – ConnectiCare, a leading health plan in Connecticut for over 40 years, announced today a new “Episodes of Care” program aimed at expanding value-based care programs to providers across the state. Through ConnectiCare’s new program, care will be organized and financed around a member’s entire health episode, for procedures such as knee surgery, chronic conditions like asthma, and maternity bundles encompassing the care of mothers and newborns. Signify Health, a leading value-based care platform that leverages advanced analytics, technology, and healthcare provider networks, will design, and administer the program on behalf of ConnectiCare.

ConnectiCare’s program has the potential to significantly expand the use of advanced alternative payment models among healthcare providers in the state. Similar episodes and bundled payment programs in Connecticut, including the State of Connecticut’s "Providers of Distinction” program for state employees, retirees and Connecticut Partnership Plan members, are helping health plans and healthcare providers deepen their commitment to patient-centered, high-quality, affordable care. By bundling services and costs together into episodes, the focus turns to how care is delivered across the continuum, offering incentives to improve the overall health of patients and plan members.

On behalf of health plans like ConnectiCare, Signify Health delivers program support services, episode-based analytics and technology, and other meaningful resources to help providers improve coordination of care, identify care redesign opportunities, and propel recovery.
