Signify Health announces improvements and new features to the Provider Enablement Platform

Upgrades to Signify Health’s  award-winning technology enhance a shared goal to improve patient care outcomes.

Desiree BrewerOur nation’s healthcare system continues to evolve toward more patient-centered, value-based care models. During the throes of the COVID-19 pandemic, providers and patients navigated new territory together as communities experienced quarantining, social distancing, and other safety measures. Providers not only focused on the health of their patients, but also their own health and that of their employees and staff. Many restrictions to remote healthcare, including telehealth, were lifted during the Public Health Emergency and, as a result, providers and patients adapted to using technology and virtual care.

As use of technology continues to be a viable healthcare delivery model, to treat patients remotely with the goal of improving patient healthcare outcomes, providers have embraced virtual care for engaging with patients. Signify Health’s Provider Enablement Platform places award-winning technology at the fingertips of providers and healthcare professionals to improve the quality and efficiency of their work.

The Provider Enablement Platform is a payer-agnostic population health and 340B referral resource. The platform provides an accessible, robust repository of tools needed to manage patient health populations. The technology takes the guesswork out of value-based care so providers can focus on delivering high-quality patient care and improving outcomes.

In the spring of 2023, Signify Health announced a new version of the Provider Enablement Platform replete with improvements and new features. To learn more, we talked with Desiree Brewer, VP of Product Management at Signify Health.

Navigating multiple patient groups in the platform and identifying which patients need the most support can be a challenge for providers. Can you tell us how the Provider Enablement Platform helps providers address that particular challenge?
We worked closely with our clients to understand the challenges they face which is why we’ve created the Care Management List to help simplify the process of providing a comprehensive list of up to 15% of their total attributed patients. This means our clients no longer have to sift through multiple lists and struggle to determine where to place their focus.
 
So, the Care Management List helps to categorize patients?
Yes, the Care Management List will help clients easily identify patients who are in similar groups. For example, it will group patients from the COPD, Heart Failure, and Hypertension cohorts and track their progress on the Performance Dashboard. This means that healthcare teams can make informed decisions about who needs outreach and when. 
 
What role does the Performance Dashboard play?
The Performance Dashboard provides a comprehensive view of a client’s performance compared to indicators of success in the Medicare Shared Savings Program (MSSP) accountable care organization, or ACO. It’s important to stay ahead of the curve in healthcare, and that can be a challenge given the changes that providers face. The Performance Dashboard is a valuable tool because it helps administrators and providers track their progress–both as a practice or group of practices and relative to other ACO participants–across several leading and lagging performance measures with links to actionable patient lists. 

The HCC Dashboard is new and something we’ve worked really hard on. We’re excited to announce upgrades that will make it even easier for clients to review their patients’ historical chronic conditions which is important to improving patient care. Our clients have talked to us about how frustrating it can be to have to click through multiple pages to access the information they need. We heard them and responded by making it possible to click on the links within the dashboard so they can get to where they want to be faster.
 
What do those improvements actually look like?
These upgrades will have a positive impact on everyday efficiencies. For example, here are the top five improvements to the HCC Dashboard:

  1. It will take fewer clicks to access the list of top chronic conditions gaps.
  2. The overall navigation is enhanced with new hyperlinks that provide more efficient ways to use the tool.
  3. We’ve increased the transparency into historical health conditions and gap closures over time which improves accountability, enhances quality of care, increases patient trust, improves resource allocation and population health outcomes.
  4. There are three new columns on the Patient Summary tab and Details Reports, providing additional patient filtering capabilities.
  5. We’ve increased the functionality for our Rural Health Clinics and Federally Qualified Health Centers by providing clickable links at the clinic level. This helps clients easily see which patients are attributed to that particular practice that may have unaddressed conditions. 
Do you have anything additional you would like to add?
Many of these new features and enhancements came directly from our clients. We took their feedback and used it to grow the Platform. I’d like our clients and healthcare teams everywhere to know that we are attuned to their feedback, and we always welcome their suggestions. These updates are designed to make the Provider Enablement Platform easier to use and to free up time for the primary care team so they can focus on spending more time with patients and improving their health outcomes.
