Taking a small step toward feeling your best can be as simple as opening your front door. Signify Health teams up with top health insurance plans to bring no- or low-cost, in-home visits to individuals who could benefit from a little extra attention.

Here’s the scoop: Your health insurance plan might identify you as someone who qualifies. Then, you will be scheduled for a visit at a time and date that works for you. Not in a clinic or office setting but in your home.

These visits, called In-Home Health Evaluations (IHEs) aren’t meant to replace your regular doctor visits. Instead, they’re a helpful add-on. And people who have experienced an IHE say they’re worth it.

Navigating health care today is difficult for everyone, but particularly for those managing complex or chronic conditions.  Fragmented and disconnected care can lead to poor adherence to treatment, not receiving care timely, increased costs and decreased satisfaction.  We are addressing these challenges for health insurance plan members who receive In-Home Health Evaluations (IHEs) from Signify Health.
