Our national network of more than 12,000 clinicians works to help people gain a more comprehensive view of their health by conducting In-Home Health Evaluations (IHEs). During an IHE, clinicians identify chronic conditions, close gaps in care, and address social determinants of health, making it possible for health plan members to get connected to the right care for their unique needs. 

Navigating health care today is difficult for everyone, but particularly for those managing complex or chronic conditions.  Fragmented and disconnected care can lead to poor adherence to treatment, not receiving care timely, increased costs and decreased satisfaction.  We are addressing these challenges for health insurance plan members who receive In-Home Health Evaluations (IHEs) from Signify Health.

Loneliness Awareness Week is June 9 - 15. This year’s theme is about reducing the stigma around this natural human emotion. Loneliness may be increasingly common but there are ways to increase connection.
