What should I expect from my visit?

Before your visit

Schedule your In-Home Health Evaluation online through the scheduling website, or by phone. After you have made an appointment, you may also get:

  • Appointment confirmation
  • Email, text or call reminder 24 hours prior to appointment

During your visit

On the day of your visit, a licensed clinician will spend one-on-one time discussing your health in the comfort of your home. They’ll answer your health questions and more including:

  • Physical examination
  • Reviewing medical history
  • Going over medications
  • Performing diagnostic and preventative screenings (as needed)

After the visit

We’ll share a summary of the visit with you and your primary care provider, if you have one. We may also help coordinate other services or connect you with resources you need.

The results from these services allow you to manage your health proactively, helping slow disease progression and prevent costly health complications. When offered by your health plan, the In-Home Health Evaluation is at no additional cost to you.
