The PHN 201 Advanced Health Coaching Training will help population health RNs, LPNs, MAs, and Social Workers assume the care of populations by taking on the role of a health coach.

 

The PHN 201 Advanced Health Coaching Training will take place at the end of the second day of each 2023 Educational Summit. This in-person training on health coaching is part of a more extensive curriculum that helps to empower you to empower patients, which improves health outcomes. The in-person training component has specific objectives related to applying motivational interviewing and health coaching techniques into everyday practice and combines it with peer connection and action planning. Seats are limited.

 
Prerequisites for Attendance

You must complete the Signify Health On-Demand: Population Health Nurse 101 course by January 31, 2023, to be eligible to attend the PHN 201 Advanced Health Coaching Training. If you have not completed the course, register for it here

The PHN 201 training is offered exclusively to Summit attendees. You must register for the in-person 2023 Educational Summit of your choice in order to register for and attend the PHN 201 training. Register for the Summit here and select the PHN 201 training session when you register.

The PHN 201 Advanced Health Coaching Training will take place at the same venues as each 2023 Educational Summit. The training will begin with lunch at 11:00am local time and will include two fifteen-minute breaks. The skills test out is required and will occur in the last hour of the training.

Continuing Education

PHN 201 Advanced Health Coaching Training has been approved for 3.5 Hours of Continuing Nursing Education. 

Signify Health is accredited as a provider of nursing continuing professional development by the American Nurses Credentialing Center's Commission on Accreditation.

Learning Objectives and Approach

This interactive training emphasizes experiential learning, group activities, demonstrations, collaboration, and inspiration. Learners will have time to ask questions and engage in productive dialogue—not just with Signify Health facilitators but with their peers. Participants can expect to leave the training feeling confident in their abilities to apply motivational interviewing and health coaching techniques in everyday practice. Participants will also share their experiences with their peers throughout the training; participating in activities and discussions that harness a sense of togetherness and best-practice sharing.

During the training, learners will do the following:

  • Review tools and techniques for successful health coaching
  • Explore the impact of Health Literacy and Social Determinants of Health (SDOH) on patient motivations and outcomes
  • Apply methods of navigating challenging patient interactions
  • Demonstrate techniques to engage patients with health coaching

Upon successful completion of the in-person training, learners will be able to do the following:

  • Demonstrate techniques to engage patients in effective motivational interviewing and setting Specific Measurable Achievable Relevant Time-bound (SMART) goals
  • Identify methods of overcoming barriers and resistance while implementing health coaching chronic care management patients
  • Demonstrate effective use of tools and techniques for successful health coaching skills

Participants will also share their experiences with their peers throughout the training; participating in activities and discussions that harness a sense of togetherness and best-practice sharing.

Our educational events are an open forum for individuals to learn and network with their peers. Event participants consistently report that the greatest benefit comes from hearing what others are doing and sharing their own experiences. Signify Health reserves the right to ask individuals who disrupt this learning environment to leave the training.
