Shannon Fanning is the Director of Post-Acute and Community Services with Reid Health. In this role, Shannon manages many care management programs such as Care Coordination, Transition Coaching, Post-Acute Transition Coordination, Community Paramedic, and many specialty specific navigation programs.
Shannon is a Registered Nurse has been with Reid Health for 17 years and has served in her current role for 5 years. Shannon has a great passion for helping patients be successful and empowering the individual to navigate the healthcare system to optimize their quality of life.
