Clinical Health Coach and Population Health Nurse, CCMH Medical Clinic
Sara spent the first several years of her nursing career working in obstetrics and pediatrics. After earning her master’s degree in nursing, she spent several years working in care management and nursing administration. Sara has been working in her current position as Clinical Health Coach/Population Heath Nurse at CCMH Medical Clinic in Denison, Iowa since 2014. It was at CCMH that Sara found her passion for teaching and patient education, especially in the area of diabetes management. In addition to working with the family practice providers at their rural health clinic to improve diabetes outcomes, Sara is also the nurse and liaison between their clinic and the Endocrinologist that provides Endocrinology services to their community via telehealth.
