Sage Nakagawa

Vice President of Episode of Care Operation, Signify Health

Sage Nakagawa is the Vice President of Episode of Care Operations for Signify Health where she is responsible for the strategic and day-to-day execution of operational initiatives for Signify’s commercial business line. Signify is a nation-wide leader in value-based care services bringing healthcare homeward. Signify’s Episode of Care clients include provider entities, large hospital systems, payers, employers, and government programs. The Episode of Care Operations team is responsible for assisting in the development of success metrics, building quality programs, provider account management and innovation, building effective partnerships, and creating clinical care redesign content.

Prior to joining Signify, Sage worked for a medical adherence company where she led major biotech/pharmaceutical accounts through patient journeys to identify and address poor adherence rates. She also worked as a consultant, helping health plans transition from ICD9 to ICD10 and before that Sage worked for a state government agency to administer a self-funded long-term care program.  
Sage holds a Master of Business Administration, Master of Science in Health Administration, and is currently pursuing her PhD in Public Health Science (Health Economics).
