Paula Price
Clinical Leader, Caravan Health
Paula is a Caravan Health Clinical Leader from Indiana. She is a registered nurse with more than 20 years of inpatient, post-acute care, and community-based experience. She is a collaborative partner on many practice transformation initiatives to improve clinical and patient outcomes.
Paula is a 2018 graduate of the University of Arizona where she completed a Master of Science in Nursing, Clinical Systems Leadership. She earned an Associate of Science in Nursing in 2002 and a Bachelor of Science in Community Health Education in 1998 from Indiana State University.
Paula has been certified as a Level 1 Diabetes Educator and maintains a certificate of competency in health coaching and motivational interviewing. She is a subject matter expert for Advanced Care Management and Post-Acute Care.
