Senior Vice President of Practice Transformation
Nicole joined Caravan Health in 2018 and leads the delivery team in their work with clients on core initiatives including annual wellness visits and preventative screenings, care management and coding. She is also responsible for advanced programs around disease management and post-acute care coordination. Nicole has over 20 years of leadership experience in health care operations ranging from FQHCs to academic medical centers and independent physician practices. Prior to coming to Caravan Health, Nicole designed the learning network for the Comprehensive Primary Care program as a contractor for the Center for Medicare and Medicaid Innovation. Nicole is Fellow in the American College of Healthcare Executives and holds two master’s degrees, one in Health Care Administration from Trinity University, another in Nutrition from the University of Texas.
