Bringing health home

Our trained clinicians spend an average of 60 minutes sitting face-to-face with members to assess and document their current health status. We take the time to perform diagnostic tests, observe home safety hazards, identify social determinants of health and, in many cases, help members get their needs immediately addressed. All information is conveyed back to the member’s health plan and primary care physicians, helping your members achieve more healthy and happy days at home.

Intelligent software

Our proprietary logistics systems ensure our clinicians arrive on time—or can contact your members should traffic cause a delay. Once in your member’s home, the documentation protocols followed by our clinicians are designed to be as non-intrusive as possible. This ensures they spend less time two-finger typing and more time engaging thoughtfully with your members, all while ensuring the highest standards for completeness and compliance. Signify Health’s team of social care coordinators, along with our vast network of community-based organizations, connect members to relevant community programs and track all interventions longitudinally to measure the impact of SDOH resolution on health outcomes and plan performance goals.
