We are excited to begin our 2024 Population Health Summer Cohort Calls! We look forward to hearing from organizations that have offered to share their insights across key population health initiatives. We will also be bringing our latest and greatest tools and resources your way. We hope you'll join us to network and share strategies.

Cohort Calls are a great opportunity to connect with the Population Health staff and Clinical Leaders at CVS Accountable Care and your ACO peers. In these calls, our staff will lead discussions on specific topics to share best practices, connect with other healthcare professionals, and ultimately meet your patient care goals.

These calls are recommended for Population Health Nurses, ACO Champions, and Practice Managers. Select which Cohort Calls you would like to attend. Topic details, dates, times, and registration links can be found below.

Proactive Primary Care

Proactive Primary Care is focused on leveraging primary care utilization to drive impact on health outcomes in value based care.  Learn from your peers sharing best practices and lessons learned in developing Proactive Primary Care Programs within their organizations.

  • Identify your High-Risk patients for more frequent outreach and office visits
  • Manage quality gaps and screenings in one visit
  • Engage patients in proactive care planning
  • Ongoing tracking and insights to track success and catch rising-risk patients

High-Risk Care Management and Transitional Care

Chronic Care Management (CCM) and Transitional Care Management (TCM) services are critical components of team-based primary care, and are shown to contribute to better health outcomes and higher patient satisfaction. Learn from your peers sharing best practices and lessons learned in developing High Risk Care Management & Transitional Care Management Programs within their organizations.

A: No, you may pick and choose the calls you would like to attend. You must register separately for each call you would like to participate in.

 

Q: Will session recordings be available on-demand?

A: Session recordings will be available on Learn at a later date, but we encourage you to attend live for the best learning and networking experience.
