Doug Jacobs, MD, MPH

Chief Transformation Officer in the Center of Medicare at the Centers for Medicare & Medicaid Services (CMS)

Dr. Jacobs is the Chief Transformation Officer in the Center for Medicare at the Center for Medicare and Medicaid Services (CMS). In this role, he helps to lead center-wide efforts to move the health care system towards value-based care, advance health equity, and promote delivery system transformation. Medicare is the nation's largest payer, responsible for more than 1 in 5 health care dollars spent in the US and responsible for covering more than 63 million Americans. Prior to this role, he was the Chief Medical Officer and first Chief Innovation Officer for the Pennsylvania Department of Human Services (DHS), where he helped oversee the state’s Medicaid program. Dr. Jacobs is a board-certified Internal Medicine Physician and has continued seeing patients throughout the pandemic.

Dr. Mohamed Diab, MBA

CEO, Accountable Care Organization, CVS Health

As the CEO of the CVS Health Accountable Care Organization (ACO), I lead a team of dedicated professionals focused on improving the longitudinal health of our Medicare beneficiaries, supporting providers, and reducing waste in our health care system. To accomplish these goals, I oversee development of value-based care strategies, clinical and financial designs, and successful execution of our ACO strategy and program.

As a physician and health care executive with over 25 years of experience leading, growing and advising regional payers, integrated health care systems, academic medical centers, and pharmaceutical companies, I am passionate about transforming the care delivery system and reducing unnecessary costs for the Medicare population. Having previously served in leadership roles with CVS Health’s ActiveHealth Management and Health Data Management (HDMS) solutions; PwC’s Health Industry Advisory practice; and Aetna’s ACO and Joint Venture solutions and New York and New Jersey regional health plans, I have managed millions of lives and billions of dollars of health care spend over the course of my career. That work has helped me develop a unique perspective on the most effective and efficient ways to achieve successful population health management models.

In addition to being a physician, I received my MBA from St. John’s University in New York.


Tim Gronniger

Chief Value-Based Solutions Officer,  Signify Health

Tim Gronniger has served as Executive Vice President, Accountable Care for Signify Health since March 2022. Tim has also held the role of CEO and President of Caravan Health, a part of Signify Health, since 2019 where he leads the implementation of successful ACO and population health initiatives. Tim is the former Chief of Staff and Director of Delivery System Reform at CMS where he led the agency’s work on drug spending issues, implementation of the new physician payment system created by the Medicare Access and CHIP Reauthorization Act of 2015, and creation of new payment models among other responsibilities. Previously, he was a senior adviser for health care policy at the White House Domestic Policy Council and held senior roles on Capitol Hill. He received a Master of Public Policy, a Master of Health Services Administration from the University of Michigan, and a Bachelor of Science in Biochemical Sciences from Harvard University.


Joel Haugen

Senior Vice President, Product Management,  Signify Health

Joel Haugen is the Senior Vice President of Value Care Solutions Product for Signify Health.  In this role, Joel oversees the Provider Enablement capabilities for Signify Health specializing in helping providers successfully manage populations in risk-based contracts.  This includes optimizing the existing MSSP business and exploring new risk-based business and services.


Aria Semani

Senior Vice President, Client Partnerships and Growth,  Signify Health

Aria is the Senior Vice President of Value-Based Care Solutions for Signify Health, focusing on client partnerships and growth.  In this role, he oversees the executive client management organization of Regional Vice-Presidents and supporting functions at Signify Health, specializing in helping providers successfully manage populations in risk-based contracts.  This includes optimizing the existing MSSP business and exploring new risk-based business and services.


Aria brings over a decade of value-based shared savings experience from Unitedhealth Group, both at Optum and UHC.  Serving in leadership roles designing payer incentives, managing embedded and interoperable care delivery programs, creating value-based solution strategies, and driving client growth, he gained a diverse perspective on various components of the healthcare landscape.  In his prior role, Aria led a team focused on delivering risk and quality performance, including payer risk/gain-share programs, care delivery collaborations, provider interoperability, workflow optimization, and prospective and retrospective program ideation and execution across 100K+ Provider Organizations and 30MM+ Lives. 


Aria holds a master’s degree in Business Administration from the University of Florida and a bachelor’s degree in Economics from the University of Minnesota.  He is a father to three, Lucia, Noelle and Deacon, and husband to Rachel, an RN BSN who inspires his elevated view of clinicians and empathy for the amazing work they do.  Aria resides in Minnesota, enjoys golfing, basketball, and making music with his family and friends.

How to Join the Outlook and Strategies Seminar


You can click the "Join Webinar" button in your confirmation email or use the browser URL. 

Joining by Computer

When joining the 2024 Outlook and Strategies Seminar by computer, click on the link provided in your confirmation email. If you enrolled and did not receive the information email one week prior to the seminar, please contact us at educationalevents@signifyhealth.com

  1. Click or tap Join.
  2. Enter the webinar ID from your confirmation email, and click Join or tap Join Meeting.
  3. If prompted, enter your name and email address, then click Join Webinar or tap Join.

 

Joining by Phone

If you are participating by audio ONLY via a dial-in phone call, enter the meeting ID from your confirmation email on the telephone dial pad instead. It is important that you associate your participant ID with your audio. There is an excellent video on joining meetings at the Zoom Support Web Site available HERE.

Joining by Computer and Phone

When an attendee joins a Zoom webinar via their computer for video and chooses to dial-in by phone for their audio (instead of using Computer Audio) they will count as two separate participants. Zoom alleviates this by providing a unique Participant ID to every computer participant that dials-in by phone for audio. When the Participant ID is used, the video and phone connections will be unified to count as a single participant. Zoom displays the Participant ID (with the dial-in phone numbers and Meeting ID) when you join a Zoom meeting by computer and select the Phone Call tab from the Choose ONE of the audio conference options pop-up window.

Attendees can then place their audio dial-in call and enter their Participant ID on the phone dial-pad when prompted. If an attendee placed their audio dial-in call before joining by computer, they can still enter their Participant ID on the phone dial-pad by pressing #, entering the Participant ID, and pressing # again.

Interacting During the Seminar

Chat  : Open webinar chat, allowing you to send chat messages to the host, panelists, and attendees.

Raise Hand Raise your hand in the webinar to indicate that you need something from the host. The host may instruct you on how they plan to use this. Many webinar hosts use this feature to know if an attendee has a question.

Question & Answer Open the Q&A window, allowing you to ask questions to the host and panelists. They can either reply back to you via text in the Q&A window or answer your question live.
