Coronavirus (COVID-19) Update

Information for Patients and Health Plan Members

If you are visiting this page, you or a loved one may recently have been contacted to schedule a home visit from a Signify Health physician or nurse practitioner working at the direction of your health plan. This visit, for which you had qualified before the recent outbreak of the coronavirus (the virus that causes COVID-19), is a regular part of your plan’s efforts to keep you healthy and independent at home. The results of this visit are communicated directly and securely to your primary care physician and used by your plan to assist in care coordination and care planning.

Signify Health has taken extra precautions to ensure the safety and security of your members and our providers. We have taken multiple precautionary measures, and will continue to do so as the public health situation evolves.

Personal protective equipment (PPE) will be given to our providers in order to conduct in-home visits, including a surgical mask and gloves to wear, and surgical masks will be given to members. Our PPE standard operating procedures meet and exceed CDC guidelines and are meant to provide maximal protection for your members and our providers.
