Information for Patients and Health Plan Members

 

You or a loved one may recently have been contacted to schedule a visit from a Signify Health physician or nurse practitioner working on behalf of your health plan. The visit costs nothing and is a great resource during the outbreak of the coronavirus (the virus that causes COVID-19). This visit is a regular part of your plan’s efforts to keep you healthy and independent at home.

With more than 55 percent of healthcare facilities being forced to cancel care visits and traditional healthcare settings posing an increased risk of COVID-19 transmission, it's essential to keep safe, healthy, and informed inside your home.

As COVID-19 continues to unfold across the United States, we want to take a moment to assure you that during this unprecedented time we remain dedicated to the health and well-being of members like you. As such, Signify Health is offering in-home assessments in geographies where appropriate and with widespread lifting of stay-at-home orders. Our highly trained physicians and nurse practitioners have expertise using proper personal protection equipment (PPE) and instructing health plan members and caregivers on how to use provided PPE.

In geographies where an in-person visit is not allowed or safe to take place, our providers can conduct a virtual visit using a phone, tablet, or laptop computer. This provides members with personalized attention and care while maintaining the distancing some require at this time. Both options provide you an opportunity to speak with a clinician about any questions or concerns you may have about your health and receive helpful coronavirus (COVID-19) education and best practices to stay safe.

The results of this visit are communicated directly and securely to your primary care physician and will assist in care coordination and care planning with your health plan.
