Leslie Rytlewski is employed with Hancock Health in Greenfield, IN. She began working for Hancock in 2016 within the pulmonary specialist practice. In 2018, she transitioned onto their chronic care management team where she was a care coordinator and the team’s respiratory therapist. In 2022, she helped develop and initiate their PAC navigator role and has remained in that position since.
Leslie attended the University of Indianapolis in their secondary education program and quickly realized that teaching elementary and middle school was not her calling but education was. She then applied for and graduated under a hybrid respiratory therapy program in 2014 and began her career in healthcare.
She brings her passion for education and love of healthcare together in her role as a PAC navigator and prides herself in her ability to connect with her community partnering SNF’s and patients.
