Sullivan County Community Hospital is a 25-bed Critical Access Hospital serving rural, southern Indiana. By identifying the potential to improve patient care outcomes with Chronic Care Management (CCM), staff and providers implemented a new program. By focusing on CCM, the hospital not only realized an improvement in patient care, but they also generated more than $1M in new population health revenue.
