The Annual Wellness Visit (AWV) is a core component of value-based care. The visit provides the opportunity for patients to proactively engage in their health and for healthcare providers to evaluate their patient's comprehensive health outlook and identify factors that could be leading to poor patient care outcomes. In this case study, staff and providers at rural Springhill Medical Center in Louisiana place more emphasis on the AWV which results in improved outcomes.
