Stephanie Jeziorski has been a Registered Nurse for 26 years. Her nursing career has included working with rural health communities serving as a Public Health nurse, Nurse Educator, CPR and CALS instructor and as a Clinical Informatics Nurse. She has also served in the role of Director of Quality, Care Coordination, Patient Access and the Diabetes, Bariatric and Nutrition department. Before joining Signify Health (previously Caravan Health), she worked as the Director of Population Health in a Rural Health Clinic, where she assisted in meeting ACO requirements. She oversaw value-based quality measures for the IHP (Integrated Health Partners,) Humana and Health Partners, as well as monitored panel management and patient attribution. She has a strong passion for Population health and ACO work and is committed to making a difference and helping others succeed! She lives in Ottertail, MN and is the mother of two boys, Jordan and Joshua and grandmother to Autumn Rose.
