For nurse practitioner (NP) Charles Moon, the In-Home Health Evaluation is more than a chance to make a difference for health plan members – it’s also an opportunity to give back to the community he’s always called home.
“I grew up close to the county where I primarily see members. I know about the local mill that closed and other issues that affect people in this area. Those local ties help build trust and connection,” he said.
As a kid, Charles considered a career in health care. A family member’s battle with a cancer diagnosis and his own frequent ear infections exposed him to the health care system. Despite his early interest, Charles initially pursued a different career path. Graduating during a recession made it difficult to find the job he wanted. While working in a stock room, he decided to pursue that dream by becoming an NP.
“I love being a nurse practitioner. It’s mentally challenging - no two people are the same,” he said. “I enjoy puzzles and that’s how I think about members. We have to figure out what they need to live a healthier life.”
A friend recommended Signify Health to Charles who welcomed the opportunity to supplement his income. He continues to maintain his full-time “day job” while making time to complete IHEs for Signify Health.
He believes in the value of the IHE, particularly the chance to spend time with members, discuss their health conditions, answer questions and refer them for support when they have unmet needs.
“We have the chance to spend time with members and really listen to their questions and concerns,” he said. “I learn a lot from them as well.”
Since his first visit in early 2024, Charles has become more comfortable with the IHE. He also finds meaning in the little moments and the chance to make a difference. During one recent visit, he saw just how meaningful the home visit can be.
“In the time between my first and second visit the following year, the member’s husband was diagnosed with cancer and passed away. She was having a hard time and appreciated seeing a familiar face for her next visit. It means so much to members that we take the time to sit with them and listen. I’m glad I could be there for her,” he said.
In addition to his local connections, Charles believes authenticity and a laid back style helps members build trust. He starts earning that trust during the reminder call, explaining his role and the IHE and encouraging members to write down any questions they may have.
That down to earth style has not gone unnoticed. A member recently took the time to share her feedback with Signify Health. “Charles was excellent. He was so professional and caring. Overall, I was very happy with my visit.”
Charles earned his bachelor’s in nursing from Liberty University and his master’s from Washington University. In his free time, Charles enjoys spending time with his family, his partner and his dogs. He also helps on his family farm as much as possible.
