When Mary Jo Bloominger knocks on a health plan member’s door for a Signify Health In-Home Health Evaluation (IHE), she brings with her more than three decades of experience, a heart for health education and a deep understanding of Iowa’s health care landscape.
Mary Jo’s journey into health care began in the small town of Bancroft, Iowa. After her hometown lost its only physician in the 1970s, a physician assistant (PA) stepped in to help the community maintain access to essential services—a moment that would later shape her professional path. “We had a PA in our town for decades,” Mary Jo says. “That model really resonated with me.”
Today, Mary Jo lives in Davenport and combines her clinical experience with a passion for education. In addition to her work with Signify Health, she teaches future health professionals. She remains deeply engaged in preparing the next generation to serve communities like the one she grew up in. “I still enjoy connecting with people face to face,” she says. “This role allows me to stay engaged and make a meaningful impact.”
She sees each IHE as more than a routine check-in—it’s an opportunity to listen, educate and advocate.
“When people are in the comfort of their home, they often open up more and feel they can ask additional questions,” she says. “I can observe things I might not see in an office, like whether someone is steady on their feet or at risk for falls.”
These visits often help members better understand how to manage their health. Mary Jo has supported members in following up on referrals, shared tips for staying active at home and clarified small but essential details in their daily routines. “One gentleman thought he was drinking diet soda, but he wasn’t—it was a regular high-sugar drink, and it was affecting his blood sugar. We had a good laugh, but it was an important catch.”
Mary Jo sees her role with Signify Health as a way to support members and their primary care teams. “I tell people, this offered to you by your health plan,” she says. “I get to spend an hour with you. When was the last time you had that kind of time to talk through your health questions?”
Outside work, Mary Jo is a proud mother of two sons—one serving in the Marine Corps and the other preparing to start college. She’s been married for nearly 30 years and looks forward to more travel and family time as she approaches an empty nest.
