Why an In-Home Health Visit Matters

Taking care of your health is easier when you feel supported. That’s why your health insurance plan works with Signify Health to offer In-Home Health Evaluations (IHEs) - a health check that happens right in your home at no extra cost. You don’t have to travel or wait in a clinic. You can stay where you feel safe and relaxed. A licensed physician, physician assistant or nurse practitioner will come to you, listen to your concerns, and help you understand your health. This visit does not replace your regular doctor visit; it’s an extra way to support your care and help keep you feeling your best.
What is an In-Home Health Evaluation (IHE)?
An IHE is a visit by a licensed clinician. It’s not meant to replace visits with your regular doctor, it’s a way to check in, see how you're doing, and help you stay on track with your health. You can ask questions and the clinician may perform some preventive health screens to identify or monitor health conditions such as chronic kidney disease, diabetes or colorectal cancer.

The clinician can help spot health issues early and talk with you about any help you may need, like food, medicine, or help with things like getting a wheelchair or making sure you’re using your blood pressure cuff the right way.
