By joining Signify Health’s nationwide clinical network, you’ll deliver meaningful moments in a person’s health care journey, helping close gaps in care and making time for meaningful conversations about peoples’ health.
As a board-certified physician assistant/physician associate with our mission-driven team, you’ll visit people in their homes, identifying clinical and social needs and reconnecting health plan members with their primary care providers and health plans for follow-ups. You’ll conduct In-Home Health Evaluations, which include physical exams, preventive screenings, plus a medication and medical history review.
We provide the tools, training and technology you need—including a Signify-issued iPad and easy-to-use platform—so you can focus on delivering quality, unrushed visits and engaging in meaningful conversations.
