How to make the most of every visit with your health care team
Being your own health advocate means taking an active part in your health care. It’s about asking questions, knowing your choices, speaking up and using the help you can get. Here are some ways to help you stay on top of your health care path to get the support you need to stay healthy.
3 steps to know your health plan
To avoid surprises, always check your health plan coverage first:
- Look at your plan booklet (the "Evidence of Coverage") for a full list of what is covered services, medicines and costs.
- Go to your plan's website and look for the "Covered Services" or "Benefits" section.
- Call your health plan using the number on the back of your card and ask them to explain your benefits and what you might have to pay.
3 ways to stay informed
Once you know your coverage, remember these things:
- Keep your own records of all visits, test results and instructions.
- Ask questions about your care, medications and next steps.
- Use available resources like home visits, telehealth and care coordinators.
Remember: Simply checking on what to do next and speaking up when something doesn't feel right makes a huge difference.
Make every visit count
To get the most from your doctor's visit, write down questions ahead of time. If you don't know what to ask, simply ask, "What do I need to know about my health?" Being engaged tells your care team you want all the help they can give. You can also use Ask Me 3, a simple program that helps you remember the most important things to ask:
- What is my main health problem?
- What do I need to do?
- Why is it important for me to do this?
Asking these questions helps you understand your health to make smart choices. A good tip is to repeat what you heard your doctor say. That not only helps you remember but it also makes sure your doctor was clear and included everything.
Ask, learn, repeat
Taking care of your health takes effort, but it doesn't have to be hard. Learn your benefits, ask questions and work with your health care team. Every step helps you stay informed and ready. You are never alone and help is always available.
"I love what I do. A member once told me that it felt like they had already known me and that means everything. Because what I do is about listening and making connections. Everyone deserves a chance to be heard."

We start by helping you connect seamlessly with your primary care doctor. After your In-Home Health Evaluation (IHE), we share our reports with them. We also make sure you get the support you need by providing follow-up and necessary referrals.
And remember, our clinicians love when you ask questions and are ready to answer them!
I encourage members to ask questions as I’m going through the assessment. I want them to be happy they saw me and be satisfied with the Signify Health IHE. I do my best to help people live healthier.

Advocacy in action
Real-life examples show why speaking up is so important. Some members have discovered serious health issues during a home visit and receive immediate care they might not have gotten otherwise.
"During my in-home preventive health visit by a Signify Health medical professional, she discovered a serious health condition that she said required immediate medical attention. She asked me to call my primary care physician right then and there, and he advised me to get to the ER pronto. Needless to say, I was very grateful to have this benefit available to me through my insurance carrier and fortunately at just the right moment. Otherwise, not knowing I needed to attend to this medical issue immediately might have led to a serious medical outcome for me."

Others had received personalized guidance and follow-up. This made them and their family members feel more comfortable and helped individuals stay independent longer.
"Much gratitude to the nurse practitioner who did a home visit with my dad (89 years old). She was professional, knowledgeable, thorough, responsive and highly informative. Both Dad and myself were left with a plan of action and a sense of direction."

It’s easy to have an in-home visit, especially when your health plan covers it. It’s a great way to receive extra attention at no extra cost.
"I had a home healthcare visit today with Signify Health. I will admit I was reluctant but pleasantly surprised. What could be easier, from the comfort of your own home. The nurse, Janelle, was very professional and had a wealth of knowledge. Janelle spent at least an hour with me and answered all my questions without feeling rushed and she sent a report back to my PCP. I respect my PCP but he never spends that amount of time with me. I won’t hesitate to repeat this home visit when they offer it again next year. Just a little buffer to help keep you proactive in staying healthy ."
