Close to Home: Creating a Member Connection Drives Shannon Peterson’s Work

Our national network of more than 12,000 clinicians works to help people gain a more comprehensive view of their health by conducting In-Home Health Evaluations (IHEs). During an IHE, clinicians identify chronic conditions, close gaps in care, and address social determinants of health, making it possible for health plan members to get connected to the right care for their unique needs.

Shannon-Peterson---croppedSignify Health physician assistant (PA) Shannon Peterson understands the importance of helping people feel seen and heard. That desire for connection has been a consistent thread in Shannon’s life, leading her to a career as a PA and her work with health plan members in Minnesota. 

 “I enjoy the time I spend with people. Most of the members I see have reached the age where they have lost family and friends,” Shannon said. “The social interaction we provide is so important and the best part of my work with Signify Health.”

Shannon first heard about Signify Health from the preceptor she met during her PA rotation. After completing her education, she pursued a role with Signify Health to gain experience while she continued to look for a full-time role.  

After accepting a hospital-based position, she continued to work for Signify Health part-time. She enjoyed the slower, member-focused pace of the In-Home Health Evaluation (IHE).

“The visit provides people with a comprehensive review of their health and the care they receive. It not only helps us catch small things before they become big health issues, but also gives members the chance to ask questions.”

Shannon approaches the IHE as a conversation rather than a scripted process. She encourages members to ask questions as they go and make the most of this one-to-one time. Sometimes, those conversations and the physical exam lead to important follow-up care.

Recently, she encountered a member who reported no longer taking his blood pressure medicines. Although he felt fine, his blood pressure was extremely high. Shannon helped connect him with his primary care provider to help get his blood pressure back on track.  

“In cases like this, you find things that require additional attention. At moments like this, I feel like we have a real opportunity to make a difference,” she said.
