When health plan members welcome physician assistant Jennifer Riffe into their homes, they’re starting a meaningful conversation about their health—one that often leads to important insights and deeper understanding. With nearly 30 years of clinical experience and a calm, compassionate presence, Jennifer supports members across West Virginia through In-Home Health Evaluations (IHEs) that prioritize trust, connection and a more complete view of wellness.
“In a clinic, time can be limited, and it’s not always possible to dive into every concern,” Jennifer says. “But in the home, I can slow down, ask thoughtful questions and really understand how someone is doing. That time makes all the difference.”
Jennifer began her career in health care as a respiratory therapist before becoming a physician assistant. Today, she conducts comprehensive IHEs for Signify Health, allowing members to share their health history and goals in a setting where they feel comfortable and heard.
She meets with a wide range of individuals, some managing chronic conditions, others looking to better understand their overall health. Regardless of age or background, Jennifer approaches each visit as a conversation focused on support, wellness and helping members in their health care journey.
“These evaluations aren’t about treatment,” Jennifer says. “They’re about asking questions, reviewing health history and identifying concerns that may not have come up during a typical appointment. It’s an extra layer of support that can really make a difference.”
That added time and attention often leads to meaningful discoveries. Jennifer has helped members recognize the need for follow-up care and, in some cases, urgent medical attention. She also regularly talks with individuals experiencing emotional or mental health challenges, many of which are conversations that are often long overdue.
“I see a lot of depression, especially among members who haven’t had the chance to talk about it before,” she says. “Sometimes a member just needs to know they’re not alone and that it’s okay to ask for help. I always encourage them to reach out to their primary care provider.”
Jennifer also sees how members’ needs continue to evolve, with each In-Home Health Evaluation bringing a different set of expectations and needs.
“You have to meet people where they are,” Jennifer says. “Every visit is different. Sometimes it’s about answering questions, sometimes it’s just about listening. That’s how trust is built.”
Outside of work, Jennifer is a loyal Pittsburgh Pirates fan who enjoys traveling to games and exploring new places with her family. She also has a passion for live music and rarely passes up the chance to see a concert. When she’s home, one of her favorite places to be is in the stands, cheering on her son at his baseball games.
At the heart of Jennifer’s work is a simple, powerful goal: helping members feel seen, supported and confident in their health.
“When a member understands that the visit is about them, and not just their conditions but their story, it shifts the entire experience,” she says. “That’s where real support for our members begins.”
