Have your health insurance card, medical history and any current lab results from recent visits handy for review.

Gather all of the medications you are taking, including prescriptions and over-the-counter drugs.

Write down any questions you may want to ask during the visit.
For in-person visits
Your in-home visit may include both preventive and diagnostic screenings.
Here are some simple things you can do to prepare for your visit:
- Find a quiet spot in your home for the visit.
- To keep everyone safe, if you have pets and/or weapons, please secure them before they get there
- Write down any questions or concerns you have about managing your health.
- We may need to review your medicines and any home testing tools (like a blood sugar checker). Please have these available on the day of your appointment.
- Because eye care is important, the clinician may want to examine your eyes. If so, your eyes will be checked with a special camera. Dilation will not be required.
- You may be asked to provide a urine sample and have a finger prick blood test completed. Drink lots of water before the visit to help with this.
- Keep taking all your medicines just like your doctor told you to.
- We may ask that a trusted adult (spouse, adult child, or caregiver) join you for the visit.
For video visits
Online video visits are a good way to talk with a Signify Health clinician from your home. It's safe, and you don't have to go anywhere! To make the most of your visit, here are some tips:
- Before your visit, make sure your device can do video calls.
- Make sure your computer, tablet, or smartphone that you will be using is charged or plugged in.
- Find a quiet place for your video call. Make sure your internet works well there.
- Have all paperwork and medicines ready for review.
Will you need any help with this visit? It may be a good idea to have a friend, family member or caregiver with you. They can help if you have problems with your device or connection.

After your visit
You may get a special care plan just for you, if you need one. We will also share what we learn with your regular primary care provider (if you have one). This helps them understand your health better at your next visit. It is important to maintain your regularly scheduled visits with your PCP.

Do I have to fast (not eat) for this visit?
No, you do not need to fast. If you need any tests, you won't have to stop eating before them.
How long does in-person visits take?
A visit can last up to an hour. This gives you and the clinician enough time to talk and do any needed screenings.
Why do I need another appointment with my regular doctor if I’ve already had a Signify Health visit?
Signify Health visits are an extra service covered by your health plan. It does not replace your regular visits with your doctor or health care team. This visit at your home helps your regular doctor understand your health more. It also gives you more time to ask questions and learn how to take care of your health.
If I have multiple in-person visits, will I have the same Signify Health clinicians?
Sometimes, but not always. It depends on who is available and the screenings you may need. If you have a different clinician, they will be able to see your past visit notes.
