Close to Home: A Conversation with Nurse Practitioner Crystal Adams

Crystal Adams headshotCrystal Adams, a family nurse practitioner with Signify Health, views her role as more than just a job: it’s a dedicated effort to meet the unique needs of vulnerable populations. The heart of her work lies in genuine connection.

“From the first moment of contact with the member, it’s critical to help them understand the value of the visit” she said.

Working with an at-risk population requires patience, empathy and adaptability. While some visits may be quick, others require extended time to address technical, cognitive or logistical barriers. For Crystal, that time is essential to ensure the member’s needs are truly met for that day.

Navigating Health Care Challenges

During her time in the field, Crystal has encountered the tough realities many people face. “When you visit with a member who is struggling to heat their home and you're cold while you're at the visit, that's one hour of your day. Some people live through that every day,” Crystal said. “You take experiences like this with you, and you want to do more.”

This dedication often involves connecting members with vital resources, coordinating with primary care physicians and, sometimes, making difficult calls when immediate intervention is necessary.

Walking into someone’s home always presents unique challenges. Often, the most difficult part is helping members recognize when their current self-management isn't enough and connecting them with the right resources — whether that means contacting their primary care provider or securing additional support to ensure their health needs are fully met.

Prioritizing Heart Health

Signify Health clinicians perform a variety of important tests and screenings during the In-Home Health Evaluation, including the application of a wearable heart monitor for eligible members. Like other clinicians, Crystal sometimes encounters members who have questions about the continuous electrocardiogram (cECG) monitor, particularly when they hear it requires a 14-day wear time.

“It’s important for us to carefully explain a test like this to ensure members understand the value of diagnostic testing compared to a standard, one-time office snapshot,” she said. “A monitor worn for 14 days may capture something that's been missed. It's important.”

When she’s not seeing members in the home or supporting Signify Health’s Clinician Advisory Council, Crystal enjoys time with her husband in their New Hampshire home.
