Before your visit
First, you can set up your health visit from home. You can do this by going online or calling us on the phone. After you pick a time, we can send you:
- A text or email to say your appointment is set.
- A reminder text, email or call a day before your visit.
During your visit
On the day of your visit, one of our friendly, licensed doctors, physician assistants or nurse practitioners will come to your home. They’ll take time to learn about your health, and make sure you’re comfortable from start to finish.

General wellness assessments:
- Physical exam
- Medical history review
- Medication review and support
- Social and behavioral health screenings
- Fall risks
- Care for aging adults (safety and functional review)
- Women’s health
Health screenings for:
- Risk factors related to diabetes
- Colorectal cancer
- Kidney disease
- Cognitive health
- Heart health
- Chronic obstructive pulmonary disease
Care coordination and planning:
- Personalized visit report and care plans
- Chronic health education and resources
- Post visit care coordination services
We will share visit notes with you and your primary care provider (if you have one). When necessary, we help you find other services or resources you need to support your health.

No, you do not need approval from your doctor to take advantage of this service.
A licensed clinician will come to you in the comfort of your home to review your medical history and medications, answer your questions and perform screenings or tests as indicated by your health plan.
Our licensed clinicians are physicians, physician assistants and nurse practitioners.
Your scheduled time and date will be shared with you, and you can opt-in to receive an appointment confirmation email, text and/or call reminder 24 hours prior to your appointment.
Please have your health insurance card, medical history and any current lab results from recent visits handy along with your medications and any questions you may want to discuss.
Yes, if you would like a caregiver or family member present during the visit, they are welcome to attend.
A visit can last up to an hour. This gives you and the clinician enough time to talk and do any needed screenings or tests.
No, this does not replace your regular visits with your primary doctor. Discussing your results with your doctor is encouraged.
We will share a summary of the visit with you and your primary care doctor, if you have one. And, you can always call our Member Services team at 1-855-319-4448 (TTY: 711). We are available from Monday through Friday, 7 AM to 6 PM and Saturday, 8 AM to 4:30 PM CT.
Part of the assessment is to better understand your living situation. We can meet on the porch or another private area. You can also have other family members or caregivers attend if you would like. If you prefer, video visits could be a good solution.
