Before your visit
First, you can set up your health visit from home. You can do this by going online or calling us on the phone. After you pick a time, we can send you:
- A text or email to say your appointment is set.
- A reminder text, email, or call a day before your visit.
During your visit
On the day of your visit, one of our friendly, licensed doctors, physician assistants or nurse practitioners will come to your home. They’ll take time to learn about your health, and make sure you’re comfortable from start to finish.

What are Diagnostic and Preventive screenings?
Signify Health In-Home Health Visits are more than just check-ups. If indicated, they may include valuable services to identify or monitor conditions like chronic obstructive pulmonary disease (COPD), chronic kidney disease, diabetes, peripheral artery disease (PAD), colorectal cancer, osteoporosis and more. The results from these services allow you to manage your health proactively, helping slow disease progression and prevent costly health complications. It's all about keeping you healthier, longer, and all in one convenient visit.
What does an In-Home Health Evaluation cost?
When offered by your health plan, the In-Home Health Evaluation is at no additional cost to you.
