Our clinicians identify chronic conditions, close gaps in care and address social determinants of health, making it possible for health plan members to get connected to the right care for their unique needs.

Overall, our clinicians help Medicare Advantage, Medicaid and certain commercial plan members gain a more comprehensive view of their health – all in the comfort of the member’s home. These visits complement an annual wellness visit with a primary care provider and focus on overall health, well-being, and potential barriers to care — typically enabling more one-on-one time with a clinician versus a standard office visit.

Making a difference

Signify Health clinicians meet health plan members where they are – in their homes – to perform health evaluations. These assessments are an important tool for primary care physicians, especially when members are isolated, lack transportation, or have other obstacles that make it difficult to go to the doctor’s office.
