Based in Los Angeles, Jaszmin recently celebrated her one-year anniversary with Signify Health. In that time, she has found purpose in meeting health plan members where they are, both physically and personally.
“Seeing members every day is rewarding,” Jaszmin said. “You get to go into the home and get an overall picture of what these members' needs are and what they have in terms of resources and support.”
For Jaszmin, that perspective is central to the work. During in-home visits, she may help a member better understand their medications, talk through what the numbers mean when she takes their vitals or identify next steps on what the member can do to improve their health.
“This is it for me,” she said. “I would not trade this role for any other role under the broad umbrella of nursing.”
That ability to meet people where they are has led to some very meaningful moments. Jaszmin recently visited a member whose blood pressure readings were dangerously high, even though they felt fine. In that moment, she helped the member understand the urgency, identified nearby options for care and encouraged them to seek immediate medical attention.
For Jaszmin, this moment reinforced the value of the Signify Health In-Home Health Evaluation (IHE). The visit can potentially uncover an urgent need or lead to a turning point that helps someone take action.
However, not every impactful moment is tied to a vital sign. Jaszmin also recalled visiting a member who was experiencing a behavioral health crisis. She quickly recognized that the member needed help beyond the planned visit.
“I had to shift to what was more important and what that member needed at the time,” Jaszmin said.
She contacted the member’s psychiatric provider, shared the situation and helped connect the member to additional support.
Jaszmin is bringing her perspective as a field-based clinician to Signify Health’s Clinician Advisory Council (CAC), a group of clinicians like her who share insights from their day-to-day experiences. The CAC helps Signify Health better support its field-based workforce and equip them with the right tools, assistance and guidance to support health plan members. As someone who describes herself as goal-oriented, Jaszmin values the chance to contribute by sharing what she has learned in the home while also hearing from other clinicians.
“It was an opportunity to express and give some feedback and share,” she said after a recent CAC meeting. “And even get some tips and pointers from the other clinicians.”
When Jaszmin thinks about what she loves most about her work, she comes back to the human connection. Sometimes, the most meaningful part of a visit is helping someone feel heard, seen and supported.
"I find meaning in the small moments, like sharing a laugh with a member," she said. "It's so important for members to feel that they are connected with the clinician on a human level."
As she looks ahead, Jaszmin is energized by the opportunity to keep improving the experience for members. When asked what excites her most about the CAC, her answer was simple.
“I’m most excited for us to have the opportunity to grow, change and continue to bring value,” she said.
For Jaszmin, that opportunity is rooted in listening, collaboration and a shared commitment to helping members get the support they need, one home visit at a time.
