Before your visit
Once you schedule your visit, you’ll get an appointment confirmation from Signify Health. You may also get an email, text or call reminder 24 hours before your visit.
During your visit
On the day of your visit, one of our friendly, licensed doctors, physician assistants or nurse practitioners will come to your home. They’ll take time to learn about your health and make sure you’re comfortable from start to finish.

Are Healthy Home Visits available on nights and weekends?
Yes, in many regions we offer appointments seven days a week, from 7 AM to as late as 10 PM. What are preventive screenings?
Your Healthy Home Visit is more than just a check-up. In some cases, based on your medical history, the visit can include valuable services to identify or monitor conditions like chronic obstructive pulmonary disease, chronic kidney disease, diabetes and more. These services allow you to manage your health conditions proactively, helping slow disease progression and prevent costly health complications. It's all about keeping you healthier, longer and all in one convenient visit.
How can I find my results from a Healthy Home Visit?
You will receive a one-page summary and action plan from your licensed Signify Health clinician immediately after your visit. A full report on your visit is also shared with your primary care provider, if you have one. And, you can always reach out to Signify Health’s Member Services team by calling 1-855-319-4448, to answer questions about your recent visit.
