Preparation for
Your Video Visit

Follow the simple instructions below on how to join your annual health evaluation ONLINE.

Instead of a clinician coming to your home, you will join a video meeting on a smartphone, tablet, or computer (similar to Zoom or Facetime).

First, do you have the right technology?

check-mark Does your device have a built-in camera, microphone, and speaker so you can see and talk to your clinician?

What to do 10 minutes before your visit:

Locate the smartphone, tablet, or computer

1. Locate the smartphone, tablet, or computer you plan on using for the video call and find a quiet place.

Double check your device's battery level and internet connection

 Open your email inbox OR text messages app

3. Open your email inbox OR text messages app so you can receive a link to the video visit. 

Keep your phone nearby

4. Keep your phone nearby with the volume on so your clinician can call you to help you get connected. 

Keeping you healthy at home

You may have been contacted by your health plan about scheduling a home health visit. During these visits, an experienced doctor or nurse travels to your home on the day and time you schedule at your convenience. They sit with you face-to-face to discuss the care and support you need to maintain good health and well-being. Based on your preferences, we can also schedule telehealth appointments rather than meet in your home.

Gather all of the medications you are taking including prescriptions and over-the-counter drugs.
