A Software Platform That Benefits Health Plans and Providers
It’s hard to reach every member of a health plan, and it’s difficult to code every condition accurately. As part of our Health Risk Evaluations (HREs), we’ve spent years developing a proprietary software platform to enable clinicians to capture data without impeding face-to-face interaction.
Our documentation software is now available to health plans and physician networks to utilize with their members. Implementing a single platform drives consistency in data capture for CMS submission and supports value-based risk contracts for providers.
Signify Office is built for clinicians and has been utilized and tested in the field through our own health risk evaluations. We utilize iOS devices to keep the experience natural in a way that doesn’t intrude on providing care.
Each evaluation is configured based on the member demographics and health history to display all items that needs to be completed in a visit, including HEDIS and quality measures, which assist in the coordination of the best care possible for the individual.
Signify Office helps health plans reach members that prefer their regular physicians conduct their evaluation in a clinical office setting, and opens a new avenue for assessing risk data from a more significant percentage of their member population.
A national, publicly traded health plan utilizing Signify Office within their physician networks has seen over 60% panel penetration and saw 29% net new HCCs in only a couple of months.
