Lesson 1: Basic Navigation

The Welcome and Basic Navigation course is the first lesson in a six-part series introducing new users to the Signify Health Provider Enablement Platform. After completion of this course, you should be able to successfully navigate the Platform, find resources that will aid in your population health programs and understand basic functionality.

Lesson 2 Part I: Care Navigation 

Lesson two part one of the five part series is designed for population health clinical staff member who are responsible for outreach and management of your ACO patients. You will learn how to use filters, pin, and move columns and much more!

Lesson 2 Part II: Care Workflows

The second part of lesson two will review care workflows. After this lesson, you should walk away knowing how to use the available patient lists in the Platform for interventions that will lead to better health of your population.

Lesson 3: HCC Tools

The third lesson of the six-part series is designed for population health clinical staff and leadership who will be responsible for closing chronic HCC coding gaps, quality gaps and monitoring your organizations progress in HCC gap closure. After this session you will understand how to use the patient facesheet and timeline, including what data populates both as well as navigate and report out on HCC gap closure impact using the HCC dashboard.

Lesson 4: Trended Key Performance Indicators, Post Acute Care and Navigating the Performance Dashboard

The fourth lesson in the six-part series is intended for ACO Champions, practice managers and population health leadership responsible for tracking and trending performance metrics for the ACO. After this course you should be able to identify areas of opportunity in cost and utilization reduction and quickly identify how your organization is performing on the key performance indicators that lead to your success in the ACO.

Lesson 5: Overview of Clinical Episodes, End of Life Care and Network Utilization

The fifth lesson in a six-part series is intended for ACO Champions, practice managers and population health leadership responsible for tracking and trending performance metrics for the ACO. After this course, you should be able to identify network leakage, assess your organization's cost associated with common inpatient and outpatient episodes and understand your organization's hospice usage.
