“I found Signify Health through a web search,” Julio shared. “I was impressed by the scale and apparent support offered, especially, at the time, for a new graduate nurse practitioner. The team called me back quickly, and it was an easy transition.”
The power of the home environment
In experiencing different environments, he’s found a new sense of fulfillment working in the members’ homes, one that contrasted sharply with his prior experience in more rushed clinical settings.
“During my rotations, I worked in outpatient clinics, I worked in emergency rooms, I worked in urgent care,” he explained. “It's just different, it's just rushed. You often don’t have the time to properly go over everything.”
The nature of his job today is just as dynamic, he shared, as he visits different places each day and meets different people. However, Julio finds it even more engaging and impactful for members’ health journeys.
“I wanted to go to people's homes because it's much more personal,” he explained. “When you’re in the home, you can see why maybe there's a compliance issue with their medication. Why aren't they taking it? Maybe they can’t get it. Perhaps they can’t afford it.”
Education and health literacy: The key to empowerment
Julio soon discovered visiting people at home wasn’t too far off from a previous role: in-home tutoring. In fact, his main focus during a visit is education, which he sees as his most vital contribution.
“Education during in-home visit means going over all the members’ medical conditions, teaching them about the conditions they have and what the medications they take are for,” he detailed. He noted that many people he sees don’t understand their medications or their diagnoses.
By explaining complex concepts simply — like what blood pressure numbers mean and why they are important — Julio empowers members. "When they actually understand what’s wrong, they can be empowered to improve and to get better. That’s the whole point," Julio stated.
The future of health care
Looking ahead, Julio believes Signify Health is on the right track, spearheading the movement for personalized ways to close gaps in care. By meeting people where they are, experiencing different environments and members’ level of needs, every day presents a new challenge.
