During an IHE, clinicians identify chronic conditions, close gaps in care, and address social determinants of health, making it possible for health plan members to get connected to the right care for their unique needs.

“The support I received during my own hospital experience left a lasting impression,” Charmaine says. “The nurses and clinical staff made me feel safe, informed and cared for. That’s when I knew I wanted to do the same for others.”
Charmaine followed that calling all the way to becoming a family nurse practitioner. Just before completing her final exam, she faced an unexpected health crisis and was hospitalized for open-heart surgery. Still, she remained focused on her goal.
“I took the exam from my hospital bed,” she says. “It was important to me to finish what I started.” She graduated with honors and passed her board exam on the first attempt.
Today, Charmaine supports members through In-Home Health Evaluations (IHEs) across New York with Signify Health. These visits allow people to discuss their health in a place where they feel at ease without the rush of a traditional clinical setting.
Many of the individuals she sees are managing multiple health conditions. Charmaine takes the time to walk through their medications, answer questions and help them better understand their health.
“Sometimes members are taking prescriptions they don’t recognize or that are long expired,” she says. “I sit with each person and explain what every medicine is for and how to stay on track. That clarity often makes a big difference in how they manage their overall health.”
In one recent visit, Charmaine met with a member who felt fine but had dangerously high blood pressure, even after taking his medication. She urged him to seek immediate medical attention. The next day, his spouse reached out to thank her. Doctors said her quick response may have prevented something far more serious.
“These visits really make a difference,” she says. “They open the door for important conversations and empower members to take action, if needed, with more clarity.”
Charmaine lives on Long Island and is proud to serve her fellow New Yorkers. When she’s not conducting IHEs, she enjoys cooking, traveling and spending time with her two children and two grandchildren. No matter where she is, her mission remains the same.
“Nursing is my passion,” she says. “I’m here to support people in understanding their health, advocating for themselves and living well.”
